High School Athletics

To sign up for 2020-21 Athletics, here are the steps to follow:

1) Email Mr. Clawson (scc@fasdk12.org) with a list of sports for which you wish to sign up for the 2020-21 school year (Fall/Winter/Spring). You can also call 288-5757 Ext 225 and leave a message with the same information if you wish. There is an attachment below with a list of sports in which students can participate each season.

2) Print the 6 page PIAA physical form (attachment below), complete the first 5 pages on your own, then get page 6 completed by a PCP, iCare, MedExpress, or anywhere else that will perform physical exams. Paper copies of the 6 page PIAA physical can also be picked up at the High School (call ahead to make sure someone will be there to give you one). Return this form to the High School in person, by mail, or scan and email to Mr. Clawson. You can also submit your PIAA physical forms to the coach when you attend your first off-season workout.

3) Print the Player/Parent Waiver form and bring the completed form with you to your first off-season workout to give to the coach. Coaches will also have paper copies if the player/parent wish to both sign the waiver at the off-season workout. Paper copies can also be picked up in the HS office. This must be submitted before students can participate in their respective sport.

4) Please be sure to read the Ferndale Area School District Athletic Health and Safety Plan (attached below) and let Mr. Clawson know if you have any questions regarding this plan.
